What we investigate, and what you receive.
Factual investigations, surveillance, incident and workplace investigations and aerial scene capture for insurers, self-insurers, employers and solicitors. Each engagement is scoped in writing at referral, carried out under NSW Master Licence 000111255, and delivered as a written report with supporting evidence.
01 / Factual Investigations
Factual Investigations
Interviews, signed statements, scene inspection and document review for workers compensation, work injury damages and public liability claims. The report records what was said, what we inspected and what the documents show, with no opinion on liability or medical questions.
What we take on
- Workers compensation claims: injury circumstances, worker status and third-party recovery enquiries, to the expectations in SIRA Standard of practice S24
- Work injury damages: the circumstances of the injury, the witnesses and the records
- Public liability claims for general insurers, to the claims investigation standards in the General Insurance Code of Practice
What you receive
- Signed statements in the witness's own words, dated, in numbered paragraphs and signed on every page
- Scene inspection record with measurements, photographs and video
- Written factual report with the statements, documents and inspection record attached
02 / Surveillance
Surveillance
Covert observation of the worker named in the referral, in or from public places only, without inducement, entrapment or trespass. Conducted within the Surveillance Devices Act 2007 (NSW) and to the expectations in SIRA Standard of practice S25, as amended in December 2025.
What we take on
- Workers compensation claims referred under Standard of practice S25, with scope and duration agreed in writing before any activity starts
- CTP claims, to Part 4 of SIRA's Motor Accident Guidelines
- Psychological injury claims, with particular care, where SIRA asks insurers to weigh the need for any surveillance against the worker's recovery and return to work
What you receive
- Timestamped surveillance log
- Video and still photographs where lawfully obtained, with other people redacted where possible
- Written report of what was observed, with no opinion on capacity or liability, stored and shared securely
03 / Incident & Workplace
Incident & Workplace Investigations
Independent investigation of workplace incidents, injuries and conduct matters, commissioned by employers, insurers and their solicitors. Structured with the ICAM method, with witness statements taken in a form suitable for use in later proceedings.
What we take on
- Incidents and injuries: the sequence of events and the contributing factors, structured around absent or failed defences, individual or team actions, task and environmental conditions and organisational factors
- Conduct and grievance matters: findings on the balance of probabilities, with stronger evidence required the more serious the allegation
- Management action: what management did, when, under which policy and with what contemporaneous evidence
What you receive
- Signed witness statements in the witness's own words, in the first person, dated and signed on every page
- Investigation report setting out the facts, the evidence relied on and the ICAM analysis
- Scene record of photographs, measurements and video, scheduled around any SafeWork NSW requirements and without disturbing a preserved site
04 / Aerial Scene Capture
Aerial Scene Capture
Sites and scenes documented from the air after an incident, flown by a remote pilot licensed by CASA. It is used to record places, not to observe people.
What we take on
- Incident sites and their surroundings, as a companion to a scene inspection record
- Public liability scenes where layout, access and surrounding conditions matter
- Sites and premises, with the consent of the landowner or occupier
What you receive
- Still photographs and video of the site, with no audio
- Imagery supplied with the scene inspection record where one is commissioned
