How it works

From referral to written report.

01

Referral

Referrals come by phone, email or the enquiry form, with the purpose, the parties, any interpreter requirement, and the claim or incident information needed for the work.

02

Scope agreed in writing

We confirm scope, method, timing and fee in writing before work starts. If part of the request is not lawful or achievable, we say so at this stage.

03

Investigation under licence

Work is carried out under NSW Master Licence 000111255, within the agreed scope and within NSW surveillance and privacy law. Evidence is logged as it is collected.

04

Written report with supporting evidence

You receive a written report with the statements, imagery and documents attached, written on the basis that the worker and their representatives may read it.

Our approach

Factum: Latin for "a thing done."

We deal only in what can be established, verified and stood behind. Three principles govern every referral.

01 / INDEPENDENCE

Fact, not a case for one side

We report what witnesses said, what we inspected and what the documents show. We do not offer opinions on liability, recovery or medical questions, and we do not build a case for a predetermined outcome.

02 / METHOD

Scoped before we start

Purpose, parties, scope and timing are agreed in writing at referral. The work stays within that scope, within licence conditions and within the law.

03 / RECORD

Written to be read

Evidence is logged with a chain of custody from collection to report. Every report is written on the basis that the worker and their representatives may read it.

Common questions

Before you refer a matter

How can I check that Factum is licensed?
Factum Investigations is licensed under the Security Industry Act 1997 (NSW) and holds NSW Master Licence 000111255.
Do you work to SIRA's Standards of practice?
Yes. SIRA does not license or accredit investigators. Its Standards of practice set expectations for workers compensation insurers, and we structure our work so that insurers can meet them. Factual investigations follow the expectations in Standard of practice S24, and surveillance follows Standard of practice S25 as amended in December 2025.
What does a referral need to include?
The purpose of the investigation, the parties involved, the scope and parameters, whether an interpreter is needed, and enough claim or incident information for the work rather than the entire file. We confirm the scope in writing before starting, and we agree timing with each referrer.
